Life Biosciences reports first human data on ER-100 glaucoma gene therapy

Life Biosciences released interim data on October 8 from its ongoing Phase 1 trial of ER-100, an investigational gene therapy targeting optic neuropathies, according to Business Insider Markets. The readout covers three participants with open-angle glaucoma, a progressive condition that ranks among the leading causes of blindness in older adults and for which vision loss is widely considered irreversible.

All three participants tolerated a single intravitreal injection paired with eight weeks of daily oral doxycycline through Day 56 without safety signals that would halt the trial. Two of the three showed preliminary improvement in visual field testing, though the trial is designed to evaluate safety and tolerability, not efficacy, so those observations carry limited statistical weight at this stage.

Sharon Rosenzweig-Lipson, Chief Scientific Officer of Life Biosciences, described the significance of the board's decision in the release: "We are encouraged by the Data Safety Monitoring Board's recommendation to escalate to Dose Level 2, a decision that allows us to keep moving this trial forward on behalf of people living with optic neuropathies like glaucoma."

ER-100 works by delivering controlled expression of three transcription factors, OCT4, SOX2 and KLF4, collectively referred to as OSK, via an adeno-associated virus type 2 vector injected directly into the eye. The OSK factors are then activated by the doxycycline regimen. The mechanism is intended to reset gene expression patterns in retinal ganglion cells, the neurons that connect the eye to the brain and do not naturally regenerate once damaged. Patients also receive an oral prednisolone taper to reduce the risk of immune response to the viral vector.

The trial, registered as NCT07290244, is open-label and covers adults with optic neuropathies broadly, including both open-angle glaucoma and non-arteritic anterior ischemic optic neuropathy, the most common acute optic neuropathy in adults over fifty and one for which no approved treatments currently exist.

Life Biosciences says the Phase 1 trial was initiated in the first quarter of 2026, making this the first human data from the program. The company's pipeline beyond ER-100 targets additional age-related diseases using the same epigenetic restoration platform, though no clinical-stage programs in those areas were described in the release.

The advance to Dose Level 2 is the immediate thing to watch. Phase 1 dose escalation decisions are driven by safety, and the board's clearance removes the most immediate obstacle to the program continuing. What remains unknown is whether the visual field signals seen in two of three participants will persist, strengthen or disappear as the trial expands and follow-up lengthens. A three-person cohort at a single dose level cannot establish whether the improvement is treatment-related, and the company has not indicated a timeline for Phase 2 planning. Investors and analysts tracking the epigenetic reprogramming space will be looking for the Dose Level 2 safety readout and any further functional data before drawing conclusions about the platform's therapeutic potential.
